Jay Eastridge obtained his Doctor of Pharmacy degree from Mercer University Southern School of in 1993. With over 32 years of experience, he has served as both a staff and clinical pharmacist at Taylor Regional Hospital, ultimately rising to the position of Director of Pharmacy and Clinical Services Administrator. Additionally, he has expertise in home infusion as the Director Pharmacy for CertaCARE. Jay has been happily married to Jessica Davenport Eastridge for 33 years, and they have three children: Lauren John, and Chloe, along with a son-in-law, Mike, a daughter-in-law, Alyssa, and another son-in-law, Chanson. They take pride in being grandparents to seven grandchildren: Easton, Hudson, Jaxson, Jett, Jrue, Audrey, and Jayce. Jay is actively involved with 3Trees Church and enjoys spending time with family, listening to music, DIY projects, and playing golf.

Danielle Smith is a dedicated pharmacist committed to her community. She graduated in 2011 with a Doctor of Pharmacy degree from the University of Kentucky College of Pharmacy and returned to her hometown of Campbellsville, Kentucky, with her husband, Adam, who is also from the area. Together, they are proud parents of two children, Addison and Dawson. Since officially joining the Eastridge-Phelps Pharmacy family in 2019, Danielle has been a familiar face in retail pharmacy since 2005. In addition to her professional life, she actively participates in her faith community at Pleasant Hill Baptist Church, sharing her musical talents by piano and singing in the worship team. In her spare time, Danielle enjoys family moments, baking, and passionately supporting the Green Bay Packers and Kentucky Wildcats.

Meet Josie, a dedicated pharmacy technician with 17 years of experience. At Eastridge-Phelps Pharmacy, she oversees the MediPlanner Program, ensuring compliance for our valued customers. In addition to her pharmacy role, Josie is a licensed realtor. She lives with her husband,, and their son, Dextin, with another child on the way.

Amanda Perez is a dedicated and experienced Pharmacy Technician with over 25 years of service in the pharmaceutical field. Known for her exceptional customer service, Amanda consistently goes above and beyond to ensure patient satisfaction.   She is happily married to Cesar and is the proudmother of her daughter, Saylor. In her free time, Amanda enjoys exploring her passion for culinary arts, particularly learning to cook new and authentic Latin dishes.  Above all, she cherishes spending quality time with her family, creating lasting memoriesand strengthening their bond.

Jeremy Hall has been a part of Eastridge Phelps since 2011. A 1994 graduate of Green Co High School, he has one daughter. In his leisure time, Jeremy enjoys riding his motorcycle and playing golf, as well as attending concerts, musicals, and outdoor festivals. He is an active member of Elkhorn Baptist Church and cherishes moments spent with family and friends. His greatest pleasure in working for EPP is the family-like atmosphere and the opportunity to connect with numerous customers during delivery runs.

Melissa Chase is a committed pharmacy expert with 19 years of experience in delivering outstanding patient care. Renowned for her skills in medication synchronization, she effectively manages her patients' prescriptions to promote adherence and achieve the best health outcomes. Beyond her career, Melissa is a loving wife and mother to her two amazing children, Myles (9) and Valorie (4). In her leisure time, she enjoys exploring the outdoors with her family, making cherished memories while appreciating the beauty of nature.

As a dedicated pharmacy technician, I bring a passion for patient well-being and a meticulous approach to my work.  For the past two and a half years, I've been a proud member of the Eastridge-Phelps family, and genuinely value the company's commitment to a family-oriented environment.  Outside of work, my heart belongs to my loving husband, Brady, and our two wonderful daughters, Cora and Teddi.  Spending quality time with them is what I cherish most in life.

A Campbellsville native, I have been a pharmacy technician for over a decade.  My greatest joys in life are my amazing children, my daughter Braysen and my son Briggs, and our dog Allie.  When I'm not working or spending time with them or my boyfriend Jonathon, you can find me exploring the outdoors hiking through trails or shed hunting.  I'm also a passionate sports enthusiast who loves the Chicago Cubs, the San Francisco 49ers and the North Carolina Tar Heels.  And here's a fun fact:  I'm a twin.

Hi! I’m Saylor Wilson, a junior at Taylor County High School. I’m involved in FFA and HOSA, and after graduation, I plan to attend Western Kentucky University to pursue a career in Physical Therapy. I love helping people, and my favorite part of working here is meeting and getting to know people in our community!
Outside of school and work, I enjoy playing golf, spending time with my family and boyfriend, Parker, and, of course, spending money! Fun fact one of the faces you’ve seen at the pharmacy for years Amanda is my mom!

I am a new pharmacy technician at Eastridge-Phelps pharmacy! I love serving the community and building connections with customers. I value being able to transform a routine visit into a meaningful experience—on and off the clock! When I’m not working I enjoy spending time with my friends and family , baking/cooking, painting , and going on hikes! I’m looking forward to expanding my knowledge in this new career at Eastridge-Phelps!

Myricle Gholston is an outgoing and community-minded professional who is excited to be a part of the Eastridge-Phelps Pharmacy family. She is currently pursuing degrees in Political Science and Criminology at Western Kentucky University and has always had a genuine passion for helping others and connecting with the people around her.
Outside of work and school, Myricle’s favorite role is being a mom to her little boy. She also enjoys spending time with her family and friends, and making memories  Known for her outgoing personality and big heart, Myricle rarely meets a stranger and believes that sometimes a good conversation and a friendly smile can make someone’s day. She is grateful to be part of a team that truly values its community and is excited to continue learning, growing, and making a difference at Eastridge-Phelps Pharmacy!
